LVH/SCD


I've been diagnosed with mild LVH of the ISV 14mm, all other measurements are within the normal limits, EF 55%, overall my other heart measurements though within the normal limits are on the smaller side, I've been told I have a very powerful heart. I'm 5'9" approximately 195 lbs. about 30 lbs overweight accordong to BMI even though my cardio tells me I'm really only about 15lbs overweight because of my built. I used to suffer PVCs and hypertension, but rarely have PVCs anymore, I'm on atenolol 100mg, cozaar 25mg BID, and HCTC 12.5mg. B/P well controlled 105/60 to 90/55 are usually my reading these days.

I read on another post recently that a 13mm wall measurement of the heart would not be considered LVH in a very large person, I thought that even 13-15mm in well conditioned athletes sometimes raised concern, most doctors have told me LVH is not a good thing even when mild.My latest ECG 1 month ago did not suggest LVH like the old ones, I know its not the gold standard in diagnosing LVH, but Do you think it is possible it could have regressed through aggressive treatment? When speaking of PVCs in the setting of a structurally normal being benign and not leading to SCD, would mild LVH like mine with everything thing else within the normal limits fall into the same category as being benign and not making one more prone to dangerous arrhythmias and SCD?

I've learned to deal with the anxieties my PVCs have caused me and still learning coping methods to deal with them even better , they dont scare me anymore like before, but with mild LVH, I still wonder and these ?s Tks

Thanks Fran, it is very reassuring reply and I'm grateful for the service provided here, I try to do the right things and follow my doc orders. Glad to hear your PVCs are few these days also. Happy Holidays to you and Evveryone also.

Hi Tickertoc! Great to read such a reassuring reply for you! Also great to know you're still almost PVC-free. Mine too for now are very much under control with a daily low dose of Inderal.

I should have wrote I was diagnosed with mild LVH of the IVS( interventricular septum ) and not ISV in first line of the post and in last line I meant to write I still wonder and have these questions. Thanks and Merry Christmas/Happy Holidays to the Doctors, Medhelp and posters/readers on this site.

It is possible, but I do not put a lot of wait in the EKG for diagnosing hypertrophy.  The gold standard is an echo.

PVCs in the setting of mild LVH does not change your overall risk.  It is true that the standard for LVH is lower in a 5' tall 100 lbs person versus a 7' tall 300 lbs person.  The problem is we all know this concept, but there is not a rigid conversion scale to calculate a number for the IVS.

At this is point there is no further risk stratification necessary -- no additional tests sound like they would add information or change management.  Solid blood pressure control is important and sounds like it is in order.  Aside from diet, exercise, a healthy lifestyle and medications, there isn't anything more you need to do.
