When should I call an ambulance for seemingly heart attack related symptoms?

My husband is 42, a smoker, with a family history of high blood pressure and as of lately a few.off the chart blood pressure readings himself. He has been having a lot of back pain problems for the past few months and has been very fatigued as well. I have also noticed a shift in his mood for the past week. He seems easily agitated and angry. Two days ago he told me that he had a very bad pain in his left upper calf muscle then later that night he started to say he didn't feel well and couldn't pin point it but if forced to he would say he felt feverish without the fever and maybe a head cold. All day today he has seemed not well and of course being a very stubborn man that doesn't like to admit when he is sick this has been worrying me. About three hours ago he asked me to run his left lower to middle back because it hurt. The thing is when I reached a specific spot he jumped and said ouch right there so I didn't worry it was heart related. Now tonight a few minutes after he had layed down to sleep he called to me and said I really don't feel well honey. When I asked what the symptoms were again he said now his lower jaw was hurting him and his head as well. The new thing set off bells for me because I thought I remembered reading somewhere that has pain not related to teeth or injury can be a sign of heart trouble. So I told him I was very worried and afraid for him and he like expected got annoyed and said just give me benadryl to help me to go sleep and some motrin. (He never ever takes medicine so this scared me more) I gave.It to him and said please honey I'm so worried, to which he sucked his teeth and said in a very curt tone good night. So now I am up terrified that he is going to have a heart attack in his sleep and It takes on a good day at least twenty minutes for emergency services to reach my home, we're on a thousand acres at the top of the highest mountain, and another forty minutes to the nearest hospital.  I am so worried I am dressed to go and I put the defibrillator by our bed along with my mom's old nitron medication and I plan on watching him all night. Do you guys think I can wait till morning to get him to a.doctor and save myself from his wrath that way or is this a call 911 and take his fury? I'm not in physical danger of course but he would be so so angry. Especially if it turns out to be a cold. Although I've got my warning bells going off right now.

Oh my gosh.  I am so SO sorry no one has responded to you. I wish I or anyone had seen this sooner to offer you advice and support!  How terrifying for you dear.

What happened?  Did he go to the doctor the next day? That seems like the best answer if he would not go to the ER that night.  An ambulance is scary for many but getting him to a doctor or ER in your own car would make sense. And if he is being stubborn, then ya, call an ambulance even if he becomes livid.  

His symptoms DO sound concerning.
