I don't think even a cardiologist could tell you what, if anything, is wrong based on your description. It requires testing to rule out anything bad going on. You have enough symptoms that warrant a workup. It will probably be an EKG, a stress test, and possibly an echo. There's also the ability to do a nuclear stress test. These are used to determine if blood flow, structures, and wall motions are OK.
Don't use an ER for diagnostics. They are skilled in stabilizing emergencies long enough to get them to people who have the right skills/knowledge. See your family GP too.
Pacemakers are used when people have low heart rates that are symptomatic and in other more complex issues. It doesn't sound like you are describing an arrhythmia that is treated with ablation.
