I used to have WPW. There is a fairly technical, but comprehensive article on ****.***. In general, it either conducts from the atria to the ventricles (forward or antegrade conduction) or from the ventricles to the atria (reverse of retrograde condution), and there are several common locations and morphologies of the pathway. If forward conducting, you may have a delta wave on your ecg; retrograde, you have concealed pathway, not visible on ecg when in sinus rhythm. Either way it can give you a reciprocating tachycardia called AVRT. Like Pika says, forward conducting with afib is not good. However, many types of WPW are easily cured with RF ablation.

I have wpw. First diagnosed and medicated at 15 yrs old. Now I am 50, I am still having my active accessory pathway. And it is only the path that conducts from the Atrial to the ventricle. It makes me NOT a pacemaker dependent. They ablated my his bundle and my av node is not functioning.
When my palpitations lasted days and nights then I would get faint (collaped) to convert to normal sinus rhythm. When I was having a fever, even a slight fever. I'll collape too. During fever, my palp will run like a loops. I think those collaped are dangerous. Once my heart was unable to restart then I might leave. Now he said I got A-Fib (might be gone) and it is no good for wpw too.
