HepB test and Liver CatScan report

I got my Liver/Abdominal/Pelvic are Cat Scan (CT) report today and following is what the report said, CAN SOMEONE please let me know how and what is the condition I am under (my HepB blood tests are below):

CT ABDOMEN W/WO IV Contrast.

Serial Images were obtained without and then following the administration of intraveneous contrast according to the triple phase artenal biphasic imaging.
Images demonstrate that lung bases are clear. The heart is normal in appearance and without evidence of pericardial effusion. The LIVER is normal in attenuation. No abnormal enhancement in arterial or portal venous phases within the liver is identified. The gallblader is seen, The Spleen is unremarkable. There is a low attenuation region in the midpole of the right kidney. This is too small to characterize and does not enahnce and may be compatible with a small renal cyst. The ureters are identified segmentally and in either entirety and they enter into the urinary bladder unremarkably. The appendix is normal. No evidence of bowel obstruction or free fluid is identified. No significant pelvic lymphadenopathy is seen. The visualized osseous structures are intact.

Following were my blood test results :

I will be able to get my self out of Hep-B ever if I follow good diet, no alchol and a good amount of excersize.
this is good for general helth for anybody but nothing to do with hbv

Heb-B from me, the doc said I am a healthy carrier and I can live longer, is he saying correct (looking at my report above)?
hbv is not so dangerous once you know and monitor/cure is never deadly

make sure you check your hbsag quantity because those with hbsag<1500iu/ml (usually inactive carriers) can get rid of hbsag with immune modulators interferon and ntz, while if higher i'd say chances are very very low

alt only have no meaning, you should monitor both hbsag and hbvdna quantity in iu/ml if both are decreasing alt 48 is your immune system killing infected cells, if both or one of both rising it is virus getting new cells infected, if both stable alt is related to food, pregnancy or other causes
also check ultrasound (liver cancer detection) and fibroscan (fibrosis detection/liver damage) if you can every 6 motnhs

is your baby vaccinated?

If your wife is still not vaccinated, get her vaccinated immediately, if she tests negative for HBsAb.

I am not sure if in USA they have hbsag quantity test available.
i just saw from hcv community there are only about 5 fibroscan in all US, compared to europe is like one of the poorest country in the wrold as heathcare so i understand why also hbsag quantity cannot be found (but silly US produce the kit)
we have a fibroscan almost for every provence, surely for every main regional hospital

look for university and research centers you will find both fibroscan and hbsag quantity, also fibroscan makes more sense since it is very accurate for no fibrosis/severe fibrosis, stages in between are not important to choose therapy or have any relevance for illness

Thanks Stefano, I am not sure if in USA they have hbsag quantity test available. I have another question looking at the above blood test reports and CAT Scan he said I am inactive carieer not sure how he made that decision (also my wife has not contracted fortunately the disease from me even after having unprotected sex, thank god).

HEPATITIS B VIRUS DNA, PCR
281412 H (REF: <29 IU/ml)

this is replication of the virus which is not correlated to hbsag quantity (hbv surface antigen)

Thank you Setfano, so is my hbsag level good enough to get rid of it after taking modulators (interferon and ntz)?

Thank you Sharp7 and Stefano, appreciate your responses.
So just to make matters more clear to me, my CT SCAN report looks good, correct? Also I am doing this treatment in US (as I am here on a project) and now I am planning to go to a different doctor (2nd opinion) before I go in for Liver biopsy. Another quick question is looking at my report do you think I will be able to get my self out of Hep-B ever if I follow good diet, no alchol and a good amount of excersize. Also to tell you me and my wife having been having sex (unprotected) for almost 3 years now and thank god and fortunately she has not contracted Heb-B from me, the doc said I am a healthy carrier and I can live longer, is he saying correct (looking at my report above)? Appreciate your advice on this,

actually i didn t have it eather and i am on cirrhosis with regenerative nodules and couple of months ago had a focal liver lesion of 6mm.
focal liver lesion has dissapered now but they used only ultrasound and said tomography is necessary only to confirm HCC in case of suspicion.

i'd say your CT scan was 100% unnecessary and to me also biospy is too much since you probably have nothing, i'd definitely wait for a fibroscan machine to be available

Did your doctor give you good reasons for a CAT scan? I ask because this seems very peculiar to me. One does not do a CAT scan unless one has specific things to find out from it. CAT scan contrast, depending on which contrast is used, could carry their own list of side-effects.

I mention all this because I know that many doctors in India are quite corrupt, and ask for unnecessary lab procedures to get a kickback from the lab... as a percentage of the cost of the test or something like that.

they will do the biopsy

biopsy can change blood tests, the piece of liver is not very small as normal biopsies because liver damage is not the same in every part of the liver so the smaller the sample the less accurate the result

also be aware there are risks with biopsy although very low 1 on 100000

Thank you bbery, but my doctor is asking me for going for a liver biopsy and also that the hospital where I have to go for liver biopsy they say I have to go for blood test first and 2 weeks after my blood test they will do the biopsy, I am confused as to why 2 weeks delay why can't they do it in the same week after a day/two of blood test. Appreciate if you have any insight on this. thank you again.

a case of a healthy carrier.. your immunse system has control of replication of the virus. liver is good. all results look good.
no treatment needed..
