Muscular weakness

Hi Stef,  Aduiski and others

As stated in other post of mine I started taking entecavir 0.50mg and it's been a month now.

I started getting muscular weakness in legs (calf muscle).  I feel the weakness and little muscle fatigue day after a cycling (about 25km).

Never had this symptom before.
Could it be the side effect of entecavir?

I  am taking 10000IU of Vitamin D3 by Solgar (it comes in soft gel caps)  for past 4 days, earlier I was taking Calcirol powder.

HbeAg negative
HBcAB positive
Viral load 6250 iu/ml, 3.8 log
DNA quantitative 7622 s /co

As on 4th March 2016
HBsAg quantitative 1666.10 IU /ml

SGPT 140 (later 69 on 12 February)
SGOT 80 (later 65 on 12 February)

US normal
Upper GI normal

Got LFT done today
Oneonta month on entecavir 0.50mg
SGPT 56
SGOT 52
Total bilirubin  1 mgs%
Bilirubin Conjugated 0.4 mgs%
Before starting treatment bilirubin was much lower

Before treatment readings were SGPT  140 and SGOT 80 on 1 February 2016 later came down to 69 and 65 without treatment  on 12 February 2016

yes it can but not suddenly but gradually that too in advanced stage of decompensated liver. I think problem with u might be thing apart from hbv. Hbv causes more mental panic than it causes physical ailment. hope ur liver function tests are within normal.

Hi SphereAj,

No..It was detected last year.But when I donated blood in 2010, there was no HBV for me. So, I assume that my HBV is max of 5-6 years old.

I used to ride 50km, 100km and more,  never experienced such weakness before. Will take electrolyte next time.  I do drink lots of water.  

I started getting these pains and fatigue in last week of January 2016,  after that coincidentally I had to go for blood test and HBv came up.  Can chronic HBv suddenly cause these problems?

this is as simple as treating  acold,  cycling 25 kms might have led u to electrolyte deficiency na+,k+ and dehydration. Check ur blood  for sodium and potassium, drink plenty of water while exercising and drink coconut water and take a little more salt. this has nothing to do with hep b or etv

When You say you got HBv 5 years ago, do you mean it was detected 5 years ago?

I have muscular pain since 20 years, I am 35 years now. I don't think it's anything to do with HBV. I got HBV 5 years back, so I feel that pain isbnothing to do with HBV. I have shoulders pain.I feel relaxed after doing exercises.

Muscular pain could indeed be caused by thousands of other issues. But hepatitis b is certainly one of those diseases that causes this problem. Just search the web, everywhere you would find muscle pains and cramps are associated with hbv. But, of course, not all hbvers experience this problem, as everyone is unique.

So,  will this pain persist or will it clear as the Viral load and DNA comes down?

@ liver patient -  is your Viral load, HBsAg reducing even without medication?

Hi,
Since your muscle pain started after you begun with the medicine, it will probably go away after some time when your body adjusts to the treatment, hopefully.
As you have noted above lactic acidosis is given as one of the side effects of entecavir; likewise muscle pain or cramping.
I have not been given quantitative hbsag test; I don't think it is a type of test they use where I live. I have only once, August 2015, taken hbv dna test, so couldn't say which way it is going now; I hope they would take for me this test again.

Yes, in your case the mascular pain may be related to your immune system being activated as Stef stated. I do not use medicines yet, but I have this special pain and weaknes of muscles; it started on my calf, then slowly expanded to my thighs. The pain is present 24 hours, and has been getting worse starting from two years ago.
Now after Stef mentioned increased lactase levels might cause this type of pain, I'm beginning to have an idea my problem could, in fact, be exactly lactic acidosis. I'm saying this because the mascular pain is exactly similar to the pain one feels after hard excercise; and I know that hard excercise produces excess of lactase buildup in our body which gives that special feeling in the muscles.
Now I'm beginning to have an idea about the problem that has been puzzling me for long time.

Lactic acidosis is one one of the sides of entecavir. Will check the lactate levels soon

I don't know if Slogan vitamin D 3 soft gel capsule is better than Calcirol powder. Calcirol comes in 60000IU sachet I used to divide the powder in 6 parts and take one part everyday giving 10000IU.

it is probably the effect of immune system activated to clear hbvdna or infected cells, keep taking vit d and when hbvdna gets undetectable and ast-alt normal the immune system will calm down and tireness will go away

check also lactate levels but increase of lactate is extremely rare and only on decompensated cirrhosis but check anyway so we have more data to point to immuen system activity as alt levels show
