The dreaded anal/rectal issues

They have reared their ugly head... Any suggestions?? We are giving him plenty of fat at pill time... I'm thinking he needs to do it 30 mins prior but dr said within 30 mins. There seems to be blood internally, I'm thinking hemmoroids and he's pushing hard stool... Sorry so detailed, but need help!!!

Hi I was in the same place three times a day bleeding into the bowl,it's a side effect I know it seems serious but it's a fact after four weeks and it felt like pooing broken glass for me they took me off and put me on to Bocepravir,It's doing the same job so far and much easier ti tolerate for some people,some
people I know put up with this bleeding from the bowel on Telaprevir,because they really want the treatment to work,so What I'm saying is they can change you to Bocepravir,but again it has it's sides as well,were still at the stage
of very crude toxic drugs,but before this as my Dr says they did;nt have much good luck

My hep specialist advised eating as much fat as I could stomach with each dose of telaprivir. So I took a minimum of 60g of fat with each dose; worked wonders! The high dose of fat has two benefits. Firstly, the drug stays in the liver for a LONG time, so really gives the virus a good beating! Secondly, because the drug stays in the liver so long, very little passes through the anal canal, result: NO BURNING!!!!  NO HAEMORRHOIDS!!!!

At first, I found it very difficult to take so much fat (6am-2pm-10pm). Sometimes I felt really sick, but never did vomit. Then I found a wonderful "fat" healthy too! "Virgin Olive Oil." There are so many different types (some flavoured) it's easy to find one that suits you personally. I would take 30g of oil with my usual fat intake. I have told other people about it, and they have all really benefitted from it. So please give it a go. Sometimes I felt so "anti-fat" that I took 60g of virgin olive oil. Nice thing about the oil is, it goes down in one shot, quick shudder, then it is all over.

Please copy and paste any peer -reveiwed research or studies done that says HCV therapy  kills healthy virus and /or healthy bacteria (good germs as you say) for our perusal..

If it were me, I would do 2 things.
1. Sunmaid make a great dried prune, it is individually wrapped, juicy, sweet. I would eat a couple a day.
2. Do some local research, what is the best probiotic in your area? Remember we are beating our bodies with these drugs. They are killing bad germs, but some good get destroyed too.
Work on making your stomach healthy, it effects the rest or our body.
And be Dr smart, share what your doing and taking.

Hang in there sweetie, it is a very hard tx but you both will make it through.
It is wonderful that you came on here looking for help.
Everyone is very kind on here and helpful.  The good thing about treating now is that there is so much information on here.  So many suggestions as to what to try.
*** Pooh said, what works for one might not work for another.  
At the same time, please let the doctor know what is going on.  I thought that my problems were normal so did not report the "shards of glass" right away.  Thinking that was what I had to suffer.  I late found out there was help for the sx.  I am not sure why doctors don't tell you that but it has been written about here...ALOT

I think what is important is to have a good arsenal of supplies and find what works for the individual. What works for one individual may or may not work for another. We all have to find what works best for each of us who has had or who gets these problems. That is why a list of various remedies is good to have. Then one can pick which one works for himself or herself. Then it is a matter of trial and error to see what works best. Plus, I found that one thing would work for awhile and then would no longer work, usually because the problem has changed some.

There is something else for which to be on alert. Some of the external anal problems may not be hemorrhoid or feces induced. Some may be the drug rash itself and may be totally unrelated to the other butt/anal problems. And, yes, I know that because I had the rash there. So it is important to figure out the exact problem too, is it anal related or is it drug rash related. The treatment of the problem will vary depending on which is the cause.

Yes, Dee, I recall those Rectal Rockets too. I never used then, but a couple who did, swore by them. LOL.

I still like Pooh's idea of not letting it get to that point by taking a small dose of imodium, the diarrhea does seem to make it worse.
I also used  yogurt as a preventative measure as well as fiber.

It's my understanding that Hydrocortisone shouldn't be used for extended
periods of time.  Desitin worked great on my children.  Cleared that rash
right up and provided comfort.  My vote goes to Desitin.   Remember the
Rx Anal Rockets?  I think it's for extreme interior anal issues.  Its been
talked about on the forum.  Extreme pain from interior anal issues - are not common.  Exterior pain is very common.   The reason I say this is because "shards of glass" would scare off even the most motivated person thinking
of starting treatment.  As you know, some people don't have the choice to wait for treatment.  Heck "shards of glass" will scare me off.  : )

Well, actually, for me, I would give the first prize to the Xylocaine with Hydrocortisone for the external Incivek issues. It numbed everything so nothing hurt and nothing itched (at least on the outside). Plus the Hydrocortisone healed the area. Of course, it did nothing for the shards of glass, LOL.

I never said I knew everything about poo or pooh. However, I had just about every side effect from Incivek that you can imagine related to the anus, rectum, butt, butt cheeks, perineum ... you get the picture. I am relaying what worked for me. And one thing I do know is that you cannot (are not supposed to) push Desitin up into the rectum, and that is where the fissures are that cause the shards of glass/razor blades. I am glad Desitin worked for your  friend, but you just cannot push it up to where the shards of glass causing fissures are. They are higher up and, anyway, Desitin is not supposed to be used internally. Even if it was, your finger is not long enough to reach those fissures.

I say try everything until one finds what works for them. But I can also say, no one wants to take too long to try to find what works for shards of glass. Shards of glass is a potential game changer. I never could have tolerated that membrane/tissue ripping shards of glass pain for 12 weeks and I am a very, very tough person with an exceptionally high pain tolerance.

On the other hand, I did use Desitin for the external rawness, pain, itching, etc. It was effective for the external problems most of the time, depending on the root problem.

What a topic, LOL. Anyone read a good book lately?

Something else I used for the external irritation and itching was Xylocaine with Hydrocortisone cream (prescription). It did work because it numbs the area. The problem can be that whatever is going on changes with these Hep C meds so what works one time may not work the next. I had quite an arsenal of creams and stuff and just used whatever worked for that particular time. But keep in mind, if it is internal, then you need to use something that can be placed internally. A cream will not cut it for an internal problem.

         i developed a  bright red rash around anus after a week of diarrhea.  my NP called it a chemical burn, and recommended desitin or a heavy barrier paste.  i used it thru the inc. phase and helped a lot        barry

If he is pushing hard stools, then a glycerine supp. would work. Ask the doc first, but they should be okay. That would get the stool to slip out easily.

If the discomfort is external, the Desitin would help, (I used Desitin on my raw skin),but if the bleeding and discomfort is internal, then he needs something to insert internally to soothe and heal that internal irritation/problem/bleeding. You should not try to insert Desitin internally. It is for external use. Annusol Supp. would probably work the best for the internal problem.

I still say try the Desitin. It really works nd a nice bonus is you're not telling the whole store that you're suffering from hemorrhoids...not that there's anything wrong with that - mind you.

A guy who works with me was complaining constantly about his hemorrhoids. I believe that he may also have had an anal fissure.

Anyway he tried everything from over the counter meds to prescription meds and nothing helped. I researched this issue and stumbled on a discussion of sufferers and the one thing everyone who tried it agreed that it worked was;

                            DESITIN!!!!!!!!!!!!!!!!!!!!  Yeah, the stuff for diaper rash.

I know it sounds crazy but it really worked for him.

Google "Desitin Hemorrhoids" and you'll be surprised.
