Undectable

I was reading a few posts about undectable,I finally got the low down on my viral load, at 4 weeks nurse said it was basiclly undectable; Here is what it says.
They have changed something about the test now. It is not resulting in the same way. Below is what we see as the result :( It is so low they cannot quantify a result--it is less than 25).

HEPATITIS C QUANTITATION IU/mL Final

HCV RNA was detectable but not quantifiable. Additional more sensitive
testing is being performed.

Comment:
The quantitative range for this assay is 25 IU/mL to 69 Million IU/mL
using a combination of Taqman real-time PCR (LLOQ 43 IU/mL) plus
reflex to another Taqman assay (LLOQ 25 IU/mL) for low viral load
samples that were detectable by the original Taqman real-time PCR
assay but not quantifiable. The limit of detection of the assay is
7.1 IU/mL for HCV Genotype 1.
What does this mean? I don't even like to see anything there...

It looks like from what your test results say you are still DET, From an earlier post you said your baseline VL was 338,000 so  you have had a excelent 4 week response ..a 4 log drop.  I wouldn"t be at all disappointed in this  result as it seems being a muti-time non -responder and with st 3-4 fibrosis ..you would have been doing the 48 weeks anyway
With this result ..it looks very positive going forward..

don't let it get you down. I was under 43 at 4 weeks and will be treating for 48 weeks. It doesnt lower the chances of SVR, we just have to do it longer to make it stay gone. Good luck to you and great news!!!!

I had the same result at 4 weeks and will be treating for 48 wks. You are still very much in the game and have had a good response. I am sorry you didn't get the UND at 4 weeks but you are not alone.

A 4 wk HCV RNA detected under 25 IU/mL and HCV RNA not detected makes the difference between treating 24 or 48 wks.  

Per Vertex:
For the purpose of assessing response-guided therapy eligibility at weeks 4 and 12 (see Table 1), an “undetectable” HCV-RNA result is required; a
confirmed “detectable but below limit of quantification” HCV-RNA result should not be considered equivalent to an “undetectable” HCV-RNA"

Undetectable at Weeks 4 and 12 patient does first 12 weeks with Incivek and P/R then an additional 12 weeks P/R only for a total of 24 weeks.

Detectable (1000 IU/mL or less) at Weeks 4 and/or 12 patient does first 12 weeks with Incivek and P/R then an additional 36 weeks of P/R only for a total of 48 weeks.

This is the same test they used in the trials, at least in boceprevir. If it was me i would ask them to re run the test. By these results its detectable but under 25.

Good luck, as for what the nurse said, i agree becuause either one is detected or not.

What they changed about the test is it now has a level of quantification to 25 IU/mL whereas previously it was 43 IU/mL.

What it is means is the PCR can detect HCV RNA from 7.1 IU/mL and above but it cannot quantify or assign a numeric number to HCV RNA it detects between 7.1 IU/mL and 24 IU/mL.  Your PCR detected a very low level of HCV RNA but the limitation of the assay does not allow for quantification.  

At this juncture it is not considered basically undetected.  Very odd that your nurse would say that.
