viral load

I posted a question yesterday about viral load going from 5million to 50 thousand. after 4 weeks. Turns out it is down to 5 thousand. I misunderstood when the office called. medications are incevik and the other 2 standards (we are new to this)  Is this a lot better or still way too high?

Could you explain what is the issue with insulin resistance?

The problem I am sure is that many of these GI's treating folks are mostly clueless.  I have one I like, but I told him about both new drugs and I corrected him on the treatment protocols for each.   He indicated he would reduce dose at ANC less than 750 and I told him to send me to a hematologist for blood monitoring.  It is great that this forum instructs everyone to research, research, research.. you have to take control and know exactly what you are talking about.. we can't count on the docs to all be pros on the new PI's.

We are not doctors of course, but I agree with Sandy and Lynda that this is too high. This is not the amount of virus in your body, this is the amount of virus in only 1 ml.

Incivek has done it's job most likely at this point so the problem is likely that the ribavirin and interferon is not working for you as well as it should. That could be a genetic issue (phenotype) an issue with insulin resistance or riba absorption or many other things.

Have you had a decline in hgb and if so where was it pre tx and where is it now? How much liver damage do you have if any?

As previously mentioned, the futility rules with Incivek state a viral load above 1000 IU/mL at 4 weeks the patient should stop all medications.  It is a response guided therapy for a reason and this is something you should discuss with your doctor as soon as possible.

Just looked at page 2 of 23 and you are correct. Had you treated? I should be starting soon, just waiting on Incivek. Has lady15 been on? I'm wondering what her doctor has told her. Thanks

I hope you come back and see this. As Sandysf says, it is still too  high. Your doctor should have taken you off treatment immediately to decrease your chance of developing resistance. This is a problem we are beginning to see with docs who have rushed patients into treatment (or patients who rushed their docs) before they understood the protocol.

Hate to be the bearer of bad news, but I thought Incivek's protocol says you have to be under 1000 at week 4 or terminate tx.    Might want to check out www.incivek.com  website.. all the info is there.
