Thanks so much Grace. Fortunately no one even asked for the order anyway and my old radiologist (not in years) moved her practice along with her tech. and everything went well! My radiologist moved because she was tired of the old doctor group kept (a bunch of guys) pushing more and more patients ($$$$$$$$$$$) on everyone including the poor techs and she went out on her own.
But I digress. I will talk to my GYN doctor. I am hoping somehow I can speak or write directly to her and not any of the nurses or office staff who probably made the error. Probably a cut and paste sloppy error of including "too much information." My GYN doctor is a very sensitive and nice person but I want to talk directly to her not anyone else so it is more likely to get resolved and also make her aware of my embarrassment with this mistake. I know everyone is trying to overcome the stigma with herpes but lets get real (I have a degree in communication science), people make judgments, stigmatize, and stereotype over things like this.

this isn't a hipaa violation.
is there a real reason for your medical history to be on this order? no there is not. certainly I'd ask to talk to the office manager at your regular clinic and point out to them how this information isn't really necessary and you find it a violation of your privacy. encourage them to have some software changes made!!
Thanks for replying. I can't see why this appeared on a routine screening mammogram. I looked up this procedure and I don't even need a prescription or order to have it done. I am going to protect myself, HIPPA, self protection by not even presenting it or deleting it from the order myself so it doesn't appear. I was so angry over feeling violated (privacy) that I ended up getting a migraine. My appt is today. Thanks again. At least I feel there is someone out there who is listening.
You have every right to go back to your doctor and ask him this question.
