herpes proctitis or something else

Male, 35 yrs.

On May 1, I had protected vaginal, unprotected oral (both gave and received) sex with a female sex worker. No activity around my anus at all. For a couple of weeks after that, I felt stressed and had red eyes, fatigue, low grade fever, stomach cramping (IBS like symptoms), itching (around lips and genital area), some burning sensation down there with no obvious outbreaks.

About 3 weeks later, on May 20, I noticed some anal wetness that caused constipation like symptoms but that came with bowel movements. I wanted to go but the stool wouldn't come out, instead only some gas would be released. With a lot of straining, some stool with mucus would come out. However, no pain or itching. Very occasional and very mild burning sensation around my anus sometimes. Also, when I strained, some discharge came out of my penis that didn't really hurt. One of my testicle also became a little sore and started hanging lower than the other as well. Went to my doctor and he told me to use Ibuprofen for testicle pain, eat more fiber and use Miralax as needed. All of which helped me to pass stool.

About 11 days after that, on May 31, my constipation is feeling better (not perfectly normal) but some fluid still comes out of penis when I strain a little. My testicle still hangs a little lower than the other, but doesn't hurt much.

Btw, the doctor knows about my encounter and has tested me for UTI, Gonorrhea and Chlamydia, all negative. Sphyllis test was also done after 4 days which was negative as well. I am planning to get tested for HIV, Sphyllis and HSV after 3 months again.

My question is:
Could this be herpes proctitis given that there was no anal activity during my exposure? I (and my doctor) did not see any lesions, sores around my genital area or around my anus with naked eye. If this could be herpes proctitis, could it show up without other traditional herpes symptoms? If not herpes proctitis, what else could it be? Until I get tested for HSV, can I have protected sex with my wife?

Please help.

A sore would appear before flu like symptoms and since flu like symptoms appear with the first outbreak, you anal issue would no be herpes from your may affair as no anal contact was made.

Thanks for your response again. When herpes causes flu like symptoms ((low grade) fever, fatigue, chills, tired eyes, etc.), do they occur after the sores start to heal, before the sores show up or during the outbreak? In other words, I am wondering if my flu-like symptoms during the first 2-3 weeks might be due to herpes or something else.

Btw, this forum has helped me a lot during the last 4 weeks to help me ease my anxiety and provide a lot of useful information.

Recurrent genital outbreaks can occur anywhere in the genital area but not from first time outbreaks. So you can rule out contracting herpes from your last affair. If you think your anal is hereps, it would be from sometime before the may affair. But even with long time hereps. they tend to break out in the same spot give or take an inch.

I have also read in various posts here that herpes can breakout anywhere, not necessarily the initial area of infection.

If I already have HSV-1 (which I may or may not), then could it be herpes related? If not, what else could it be then?

Hi, this would not be related to herpes from your affair as first time outbreaks occur at point of contact like your pubic mound area. So if no anal contact was made you can rule out herpes with your anal issues.
