It isn't herpes, and it doesn't sound like molluscum, but it does sound like something you really need to get checked out by a doctor. It could be a folliculitis - infection of the hair follicles. Since there are so many, you might need an antibiotic to clear it up.
WIth your boyfriend having genital hsv1 ghsv1), please listen to me about how you all are doing a bunch of things you don't need to do. I don't know if they are things you asked him to do, or things he insists on, but the precautions are way overboard here.
Ghsv1 rarely sheds, rarely recurs, and therefore, rarely transmits. If he's had it longer than 2 years, he sheds on about 4 days a year. Without shedding or symptoms, he isn't infectious, meaning you can't get it.
Even if it hasn't been 2 years yet, these are the shedding rares:
People with new HSV 1 shed virus on 13.7% of days (about 50 days/year), and at 11-12 months out, 7.1% (about 25 days/year) and at 2 years, 1.3% (about 4 days/year).
If you have hsv1 orally, you can't get it again genitally. You would have antibodies that protect you from getting it again in another location.
So I don't know who's insisting on him keeping his underwear on, but that's absolutely not necessary.
So get these bumps checked out - any urgent care or doctor can help you. If you have seen a derm or a doctor about the molluscum, that doctor can help you with this, too, probably.
