That sounds like an active infection. Get to the doctor, via urgent care if possible, ASAP. Although extremely rare, rivers and lakes can contain harmful bacteria that can enter through small cuts and scrapes, and cause very fast-moving infections. Best to be evaluated right away.

Yes I agree. You can get some aggressive bacteria in fresh water. Please go to the ER.
I am a clinical microbiologist and have seen some bad things from fresh water infections.
