Both kidneys shows complex thick walled cyst

My Dad is 54 year old smoking since 30 years and got severe head ache and taking Saridon tablet since around 20 years. He got head ache and when consulted doctor he adviced the following tests and here are the results.

CT Scan brain - normal.
ECG - normail.
No BP or Suger.
Kidney Function Test :
   Urea - 44 mg/dl
   Serum Creatinine - 1.8 mg/dl

Ultra sound abdomen Result says : Both kidneys shows complex thick walled cyst, more so on left side. Right side upper pole region cyst measures 5.9*4.1cms. left side upper pole anteriorly complex cyst measures 7.8*5.1cms

Please let me know is it dangerous ? Is it a kidney cancer ? Is it possible to cure this desease with medicine or surgery ?

Now after 3 months we did a PET scan and the chest scan report says few scattered Bilateral nodules in lungs with bigger one sized 7mm. my doctor suggested a re-scan of chest after a month to confirm the change in size of nodules.

Is this could be the sign of metastasis ? if yes which stage ? what treatment we can go for now ?

Hello and hope you are doing well.

Understand your predicament and sympathize with you. Histological grading will help to assess the prognosis, according to cell type, amount of foam cells, presence or absence of vascular invasion, nuclear grade, and pathologic stage provides useful prognostic information. But there is better 5-year survival rate of papillary RCC (overall and for stage I tumors) compared with that of conventional RCC, so it suggests that it is a tumor with lower malignant potential.

We did a CT scan and found 8*5 cm complex & 7*4cm minor cyst in the right kidney and 7*3.5cm minor cyst in the left kidney. We did a partial Nephron sparing surgery for the right kidney and removed the cyst.

After the test its been confirmed that  complex cyst contains papilliary RCC.

Doctor suggested a PET scan after 3 months, its been a 1.5 months already since we did the surgery and now my dad is facing occasionally giddiness after the sleep and some time minor head ache.

I am really worried, could you please advice me on any other surgery/medicine available ?  
from the description what stage/type the cancer is ?
Is this papilliary rCC is curable at this stage ?

Hello and hope you are doing well.

It needs to be seen what the cysts are due to, it could be due to an infection. Others which can cause similar pictures are tumors, and sometimes hemangiomas. Depending upon the echogenicity, the borders and the shadows created a near correct diagnosis can be made with imaging studies. If there is a doubt then a kidney biopsy may be needed.
This is again correlated with the renal function tests. The serum urea and creatinine levels are raised. The cause needs to dealt with for the levels to normalize.
