Can a blood disorder that causes clotting be the result of kidneys failing?

My hubby was first diagnosed with renal failure when he was 30. (he is now 40). The blood clotting started when he was about 23.  The doctors could not give him a proper answer as to why and never tested his blood, even with the build up to the transplant, for any issues. He was on dialysis for a 18 months before his dad gave him a kidney and now, 9 years later, that kidney is failing as well.  He was in hospital for a month in Jan and landed up in ICU with blood clots in the lungs.  He was given 4 blood transfusions but that has made no difference.  He is on warfarin again (it was stopped last  year Sept when he landed up in ICU with a nose bleed and got a blood transfusion, the warfarin was then stopped).  Could it be his blood causing all these issues?  He has an enlarged heart - could that also be a contributing factor? We have since moved him from the doctor he was with for 13 years to a new doctor and after being on dialysis for two months, the new doctor has started taking him off his anti-rejection meds and the antibiotics he was on. He has gone down from 38 tabs a day to 11 a day.  Has anyone else out there had the same or similar issue in this line? I feel so  helpless as I can't go through the dialysis for him and he is really battling with that (he battled the first time as well).  There are so many other issues he has but that will be for another day.
