Hydronephrosis

hello.

i am a 45 year old male and have had high B/P for a couple of years. my gfr was around 55 so was sent to kidney expert.

the ct showed signs of Hydronephrosis and although my kidneys did drain they took a long time to do so (radiation ct scan) so i was sent further to urology.

urology have discovered i have 2- hutch-diverticulum, mega ureters and rat tail ureters.

under the vcm they found no renal reflux but my pressure graph was unusual.

The pressure in my bladder was minimal until i was near full then it shot up when full and voiding. for this they tried botox but the pressure was showing over 60 cm of water.

the suggested course of action is the following:

remove ureters from bladder, join into one and reattach to bladder

bladder augmentation.

im ok with the removal of the ureters, but am not sure about the augmentation. my Dr has suggested i may get reflux when the constriction of the ureters is removed as i get this bladder pressure (think he called it high end detruser )

my questions are:

if im getting no reflux as of now,would it be the case i would start? i do have signs of kidney damage which they initially thought was due to reflux, so could it just be urine is trapped in the ureters and that is causing the damage,so when the constriction is gone  this urine will no longer there to do damage as it will drain away nor,ally?

could it be the pressure build up is to do with the constriction in the ureters and could cease if the system is not under the constriction anymore?

would having the hutch-diverticulum removed help with the bladder pressure?

could some sort of anti-reflux surgery be performed instead of augmentation?

are there any alternatives to the augmentation?
