9 y.o. boy with elevated ALT

My 9 year old son (70 pounds) went to the hospital with severe abdominal pain in his upper right quadrant.  Labs showed elevated AST and ALT (71 and 359 respectively.)  Bilirubin was 1.2, but he wasn't jaundice.  CT scan showed a "hot" gallbladder.  Sent us to Children's Hospital for probable surgery to remove the gallbladder.  Ultrasound showed gallbladder was fine but there was fluid around the liver.  Another scan was done (don't remember name of it, checks the dye going through the gallbladder, liver, etc.) and nothing out of the norm was noted.  Xray was done and that was fine, too.  Docs said he was constipated which I didn't argue with, but still don't think that is the answer to the elevated liver function.  Tested negative for Hep A, B, C, mono, Lymes, Alpha antitrypsin (sp?).  Liver function was tested in 2007 when he was hospitalized for pneumonia.  AST and ALT were 132 and 391 on day 1 of his hospital stay, and 66 and 248 on day 2.  Wouldn't you think the liver function should have been addressed in 2007?  Energy level has been very low for a month.  He has also had frequent and sometimes severe headaches.  Following up with pediatric GI on Monday, but I am so worried they won't figure out what has caused this repeated problem with his liver function.  I haven't overdosed my child on Tylenol, he hasn't had any injuries (other than when his 4 y.o. brother hits him in the stomach) and he is not a raging alcoholic.  Another note, tested negative for Hep B antibodies in 2007, we repeated the series and tested again for anitbodies which were present.  This time around the Hep B antibodies are NOT present and we need to repeat the series again which I am waiting until I talk to the GI doc about it.  Any thoughts?

Bloodwork came back.  Levels are still elevated, but have improved after 3 weeks.  We are retesting in 2 months.

Test shows that the ALT, AST, and GGT are high.  The GGT is a new test to me.  Does it sound like these levels could normalize in two months time?  Is it possible that a virus has been messing with his liver?

This time around his potassium was a little low (3.8) but I figured that was because he vomitted for 10 hours a couple of days prior.  Carbon Dioxide was a little low at 21.  Protein 7.8, I can't find the actual number for his WBC, but it was ok.  I think it was 6.1.  

I don't think they did check him for autoimmune diseases.  I just wrote that down so I can ask the GI doctor.  Thanks!

I am sorry that your child is ill. Have they checked for autoimmune diseases? How is the rest of his blood panel?
