Porta hepatis lymphocele

I don't have a diagnosis yet, so not sure which section to post in. I had my gallbladder removed in 2008. In 2010 I had an MRI mrcp for abdominal pain and lfts crazy high. There was fluid at the gallbladder fossa. I was told not to worry, it was normal. Lfts went down. However, through the years I've continued with some pain, and my lfts are all over the place, but seem to get higher each episode, and never return to normal. My new primary care said this wasn't normal which leads me to now.

My current MRI mrcp lists "small fluid signal intensity structure is noted along the anterior caudate love of the liver with no definite evidence of direct connection to the biliary tree". " small cystic appearing structure within the porta hepatis most suggestive of lymphocele versus seroma or focal ductal ectasia".  ERCP was suggested. I've had an EGD already.

I have an appointment with hepto in August. GI is talking of doing the ERCP now.

Any advice? Go for it, or wait for hepto?
I've no idea what this is or what could possibly be going on. Thanks.

You'll definitely want the hep to consider things for you. I'd call the hep's office and at least talk to his or her staff on whether or not this needs to be moved on before the Aug appt. They can have the hep get back to you in a few days (email, etc) even if a direct conversation wasn't possible. The ERCP procedure isn't without it's own hazards, especially of the bile duct system maybe compromised in some way. Good luck.

I don't want to alarm you with speculation based on minimal data and we aren't doctors on this site of course. But, I'm sure they would like a closer look at the area and get a possible biopsy if warranted. Since your gallbladder is out but you've still had what appear to be intermittent bile flow obstructions with a focal anomaly in the MRI data, having a hep involved is really the right thing to do here. Let us know how things go or you can send me a message as well directly. I have a great deal of knowledge in this area unfortunately due to my little liver disease pleasure.

Thanks! New territory here and I don't even really know what the implications are or could be for this thing as I'm not entirely sure whet the heck it is.
