Itchy circular lumps on testicles and itchy hard lumps on penis cap

Thanks for being there for us.
I've had some circular, itchy lumps on my testicles for close to 10 years now. Recently they started spreading (I can't help bury scratch) with one on the on the left side of the "neck" of my penis.
Again; recently, a couple of days after sex, two very itchy bumps (one large and one little developed on my penis cap and the whole penis neck/head are is really itchy.
I'm willing to send pictures to help the diagnosis. Please help. What are these and how do I treat.
