Left scrotal pain

For the past few months, I have been suffering from pain on my left testicle. Pain usually starts when I'm standing. With prolonged standing pain elevates. Sometimes I found that the pain reduces when I sit. Also, I noticed that wearing underwear will reduce or give some relaxation from the pain. At times, when I feel the pain, I use my hand to lift the scrotal sack bit up, so that i get some relief. I'm married and doesn't have any problem in sex life due to this . I visited 4 different urologist, during this period of time, and each of them having conflicting observations. Ultrasound has been recommended twice by different doctors. First time, report mentioned about bilateral varicocele, which is more prominent on left side. But there were not even a single sign of varicocele on my scrotum. Same doctor suggested, a sperm examination, and found that there are no issues in the count, but told me that morphology is slightly less, which could be improved by varicocele correction. For second suggestion, I visited another experienced urologist who completely ruled out the observations and the ultra sound results from the first doctor, and I was told there are no issues at all, I should masturbate regularly and recommended a medicine which improves circulation , which I didn't take. I still carried the pain for few more months, seeing that there is no relief, visited the third urologist . He also ruled out the varicocele theory put forward by first doctor and asked for a second ultrasound . This result never mentioned about varicocele, this doctor couldn't find anything wrong and suggested me to do yoga. It's two three months after that, and I still don't have any cure from the symptoms. This pain has completely degraded my quality if life and has become a mental pain as well. Please suggest what could be wrong . Expecting an answer

Hi,
   When you have been examined by three doctors and two of them found nothing wrong with you, then most likely all that you need to do is to use a tight under garment while you are actively moving around.This will most likely solve your problem.
However, varicocele may be still lingering around, though may be in its early stages, and needs to be monitored if the pain continues in spite of locking the testicular motion. Also doing Yoga at this stage might help to prevent its occurrence. Note that we are talking of varicocele of the veins draining the testes and not the scrotal sac, these are internal to the scrotum and will not be visible externally.
Testicular pain may also be caused by a pinched nerve that connects to the testes, in which case, the source of the problem may be your lower back, you may need to check this aspect also with a neurologist, especially if you suspect any spinal problem.

Hello,
   You seem to have done all the necessary tests and all that you need to do is to use a tighter under garment while at work. MRI should have shown any real problem with the nerves. May be you can do sitz bath in luke warm water  for about 15 minutes once in a while to relax the muscles in the pelvic area and do some Yogasanas under guidance from an instructor. If you do long distance cycling, horse riding or two wheeler riding, reduce those activities.

Thank you very much for your response. I have consulted a neurologist also, as per the advice one of the urologist I visited, suspecting a possible nerve compression. As I had the history of back pain till one year back, though I'm completely free from that now. An MRI was performed on the spine, and nothing wrong was found. Is the MRI of spine would be enough to identify a possible nerve compression toward scrotum? Also sometimes I feel a kind of numbness and slightly burning sensation on my left scrotum and neighboring areas. Also, please let me know, how a nerve compression can lead to pain while standing, which is one of the primary symptoms of varicocele? Expecting your reply. Thanks in advance
