Passing semi-hard substance when urinating - what is going on?

Hello,

I am 29, male, and in generally good health. I don't smoke, I exercise regularly, and in general I eat relatively well.

For the past few months I've been experiencing some issues and discomfort. Specifically, I have had some pain in my lower right abdomen - more akin to pressure - and some pain at the end of my penis and in the urethra. It is infrequent and not that painful, but still concerning due to its longevity. Most of the time I pass urine without any problem or second thought, but occasionally I pass semi-solid bits at the end of beginning of my stream. These are small, gunky, and stick to the side of the toilet bowl. I definitely feel them being passed, and it does not feel great. They are not hard, and are more mucus like than anything.

I went to my GP early on and thought I had a UTI or something. Swab test came back negative, and got recommended to check for STDs. Those tests also came back negative, which is what I expected given my sexual history recently (one sexual partner in the past six months, one time). So I went back and got a second urinalysis done and it also came back negative.

Unfortunately this has all been happening during lockdown so its been hard to get tested, see my GP, etc. I have a call scheduled but they aren't really taking any customers right now. Looking online I've been disheartened to find nothing about passing semi-soft stones. It is always about kidney stones and defining characteristic of those are very hard and very painful to pass. Mine are mostly soft and sticky, and while painful still relatively low on the pain threshold.

It is very infrequent - probably only happened a few times. Can't point out If I am doing anything specific - maybe a rigorous exercise, but really it seems random. I take some vitamins, but have always done that.

Only relevant medical history is I do have phimosis, which I was planning on finally dealing with this summer (circumcision) due to the steroid creams being ineffective.

Apologies for the wall of text but hoping someone has an idea. My searching online just leads me to believe I am dying. Looking up your symptoms is a terrible idea, so looking for someone who may have experienced this.

Sounds like this has been hard to diagnose since you've seen a doctor and they haven't yet.  But they did rule out the main things, so that is good.  I found this article.  https://www.healthline.com/health/sediment-in-urine#1  They are calling matter like you describe 'sediment'  There are  some underlying conditions associated if this is the case that would need to be ruled out. Some will be easy to do that because they usually have other noticeable symptoms.  Things like getting your sugar checked to make sure you don't have diabetes, are important. Biproduct in urine looks like sediment if diabetes is the underlying cause.   Checking your sugar levels will be easy to do after lock down eases up (hopefully very soon).  

Another common cause is actually just dehydration.  How is your hydration going?  You can work on that yourself while you wait to go get a physical.
