Penis Problems. Please read.

Ok first off I had bad hygiene when it came to my penis growing up. Up until I was 15, my glans were still connected to my foreskin and smegma was everywhere. I (extremely uncomfortably) told my parents and we got a doctors appointment set up. While waiting for it, I stretched loads and lots of it disconnected. By the time the appointment rolled around it was almost completely detached. The doctor prescribed some steroidal and antifungal cream and that cleared it up.
That’s the backstory anyway. Since then I’ve had a few more problems I wanted to discuss.
Along the back of my penis (kind of along the urethra but it’s on the skin) is a long, mild rash that causes no discomfort or itching, it’s just visual. I think it might be because it’s sweaty there but I’m really not sure.
Another problem is fordyce spots. I’ve researched them and they are harmless yet they are everywhere. All over the testicles and even extend very high up the shaft. The odd time a bad one might stand out as being slightly bigger than others but not enough to cause discomfort.
My last problem is the glans. After showering (using soap, rinsing and drying) the head is still kind of dry and sticky and difficult to roll back. It doesn’t hurt, but it’s not pleasant. No matter how well I clean down there, smegma can still appear on the glans after masturbation, tiny pieces only. Also, when I pull back the foreskin, on the inside of it there are small, what appear to be, open pouches of smegma. They have a tiny amount in them but they can be cleaned a bit but never really fully. They don’t hurt either it’s just weird to have them and I fear it might affect me sexually.

I am 16 years old, uncircumcised and fairly athletic. My diet isn’t great but not terrible, however I don’t think this is the case. I think it could be a mixture of a few problems. Balanitis it could explain the dry stickiness of the glans, but I’ve tried using the anti fungal cream I got before but it only gave very temporary relief, and it seemed as if the cream became smegma, despite me rubbing it in until it disappeared. I’m not sure I can do much about the fordyce spots unfortunately - they just look really weird. It’s like acne on my balls and shaft but colourless. I’ve scoured the internet looking for a solution or even some sort of recognition as to what the rash is, but I couldn’t find anything. It’s mild, and almost disappears completely with an erection but it is definitely noticeable to touch - it feels like a different texture to the skin, softer maybe.

I really hope you can help me out with some sort of advice. Thanks in advance.

First, I commend you for being so articulate and in tune with your health at your age.  That's terrific.  I always encourage young people to try to think of a trusted adult they can talk about these things with because ultimately, they can get you the proper care you need.  But I am happy to give you some thoughts to consider.

Yes, this could be hygiene related.  It sounds like you have a history of issues from that type of thing and the sweat most certainly contributes.  This does NOT itch, correct?

Previously, the cream they had given you helped.  It may again.  This article talks about penis health.  It has a large list of tips to maintain that health.  https://www.healthline.com/health/penis-health  It gives very explicit information on how to wash your penis.  This is another article on cleaning of the area.  https://www.nhs.uk/live-well/sexual-health/how-to-keep-a-penis-clean/  You note sweat as an issue and it is important to frequently take care of that.  Maybe more frequently that you currently have been. People can also get pimples on their penis.
