Penis ripping making the hole bigger

my son  is circumcised, but that dozen’t really have anything to do with the problem. The hole at the tip of his penis that the urine, and what not comes out of it ripping. The hole is getting bigger and it hurts when it gets brushed up against and sometimes when he urinate. It looks normal at quick glance but you can see that it is tearing under closer examination. he is  really concerned because it is ripping more making the hole bigger. he know it’s not suppose to rip like it is and he is not sure what to do about it  if it will get better on its own. please help concern parent Thanks

"Ripping" and "penis" are two words that I never want to have anywhere near eachother, and I would never assume the problem would get better on its own.

It might be a "normal" complication of circumcision (and if ripping is a complication of circumcision, yikes, why is nobody ever warned about this?) -- or it might be irritation from an infection that needs to be treated (and the sooner the better), or just irritation on its own that a cream will soothe.
