Scrotum tightning, Penis pushing foward

For several years I have been dealing with an issue with my scrotum tightning for no apparent reason. It happens quite often and occurs at random times during the day. When it occurs, I feel a kind of tingeling sensation and my scrotum muscles tighting almost like a ball and the skin thickening. It isn't painful but it is very uncomfortable.

The main issue is that the scrotum pulls up so high that it causes the penis to push forward.  

Basically, instead of the scrotum hanging in between my legs and the penis resting at a 90 degree angle, the scrotum tightens and at the same time pushes the base of the penis upwards causing the penis to hang at a 45 degree angle. This causes a huge discomfort because it feels very tight down there when wearing any kind of pants and even worst it causes a very noticeable bulge.

I've had 2 ultrasounds and seen the urologist twice and nothing came out abnormal.

But during both visits the scrotum was hanging normally. On the second visit I actually showed the urologist pictures I took so he would understand what I was talking about. And he admitted it was weird. He even asked a college what he thought about it but both couldn't offer any solutions. He basically told me that the scrotum is made of a ton of tiny muscles and there is basically nothing he can do about them pulling up so tight.

This has made my life a living nightmare for several years, causing embarassing situations, dirty looks, having to wear longer shirts so it covers up a bit whenever there is a noticeable bulge. And now I've come to the point that I can't take it anymore. I am desperate for a solution.

I've tried to pin point the exact moments it occurs but it is very random. The tighteness occurs pretty often and at varying levels during the day. But it does happen without exception before bowel movements.

I don't know what caused it. I might have stretched a groin muscle while working years ago that could have affected the nerves or scrotum muscles, I have no clue.

I would really appreciate someone's input on this.

You are probably being told it is normal.  You will read all over the internet that this happens and urologists are going to tell you that.  UNLESS you have pain or are having difficulty conceiving a child due to a raised temperature of the testes, you are going to be told this happens.  It's different but not a problem.  It's a change but not abnormal.

But you are looking for answers.  They do relate things like diabetes and even low blood sugar to this happening more frequently.  Mechanism of action is something I can't explain about that but am just saying it has been correlated in the info you can find on this.   The endocrine system is your hormones---  and hormones are directly related to your testes and what goes on with it.  A symptom of low estrogen, yes, men have estrogen, is tight scrotum. Thyroid issues can be related as well.  Overall, lab work to look for any abnormalities may be a further window into your personal situation.  Otherwise, there will be nothing for anyone to go on. And you'll be told it's just normal.

So, ya, hope someone else chimes in.

I think you do not truly understand the issue here.

The urologist never said it was "normal". Like I said, he found it really weird that the scrotum would pull up so high but unfourtunately could not offer any solutions. At the time that he checked me out the scrotum was relaxed and did not show any abnormalities. but I showed him several pictures I had taken and that's when he understood and even consulted his college.

The fact that i'm not in serious pain does not mean it is normal. It is NOT normal and it is very uncomfortable.

It might not be considered a "problem" to a particular doctor, but that's because he has no solution to offer.

I will look into what you are saying although I doubt it will fix the problem. The problem isn't only the scrotum tightening, but mostly the extent it tighten's and how high it pulls up. It has nothing to do with the scrotum contracting do adapt to temperature, it pulls up much higher.

But hopefully some more suggestions will come. I really appreciate your effort regardless.
I wanted to add something. I noticed that everytime I drink alcohol/get drunk the issue isn't present. I get no tightning of the scrotum.

It might sound stupid, but do you think there is something in alcohol that affects hormones levels or something of the kind?

As for the lab tests, I will probably need to ask my doctor. Thanks

Do you take any medications such as propecia?  Did they do a hormonal check on you (lab work to check hormone levels)?  Does diabetes run in your family? Just things to look at.

I would say that many men have this scrotal tightening as part of a natural reaction and it's normal.  Things like changes in temperature can influence it.  
The good news is that the doctor you talked to about it wasn't concerned but I'm sure you'd like to find an answer.  Let's see if anyone else responds.  I'm interested too!

I have never taken Propecia or anything similar. As for diabetes, I don't think it runs in the family. Never checked my hormones. But i'm not sure what correlation diabetes or hormone levels could have with this problem? Just curious.

What I can tell you is that it definitely isn't normal. I've never had this issue before it suddenly started several years ago.

No idea what caused it. I might have stretched a muscle or something without even noticing? Maybe some nerve damage? Something that could cause the scrotum muscles to be overactive or try to protect themselves? I honestly have no clue.

It also has nothing to do with the normal temperature control as it occurs even on a hot summer days.

Like mentionned above, the scrotum pulls the testicles so tightly up/forward that it actually pushes the base of the penis. Basically the whole package protrudes forward. It's really hard to describe without an actual picture but hopefully it's clear enough.

I appreciate your answer still.
