Do you have long periods of arousal without ejaculating, that seemed to be my problem with similar pain you mention, I would have maybe 2 or three days of prolonged arousal for hours at a time,e dging to almost climax, and think maybe a build up of semen in tubes was the cause of the pain, but when i masterbated to climax the amount of heavy lumpy semen that came was instant relief, and the amount that came was loads more than a normal cum, maybe not your problem tho, but thought it worth a mention.....I now know when to cum rather than have long sessions of play and edging to near climax, then can get rid of pressure in tubes which seemed to be the cause of such pain, I never had professional advice on the matter as i was too shy to explain my sexual habit..and fear of physical examination .
