need help with spinal tap results?

Everything was within normal range except for the rbc level. In tube 2 it was 13 and in tube 4 it was 11. The color was clear and the protein level in tube 2 was 27 (normal) and tube 4 was 25 (range 12 to 60).

The IgG test said it was performed and the values within normal range, but the sample was hemolyzed.

Any ideas? Traumatic tap?

Most likely the trauma from the tap. That isn't that high a value for RBCs.  Hemolyzed means that RBCs had ruptured or "lyzed" in the tube.  On the IgG test, tha means the results may be iin question if vascular IgG contaminated the CSF IgG.  Since the levels were normal, it may not be an issue.
