help diagnosing nephrotic syndrome

I've been trying to help a family in Northern Thailand with hospital travel. The daughter is late teens and has massively swollen feet, a swollen abdomen and somewhat rounded face. She can't walk anymore without assistance. I suspect the doctor treating her doesn't have the specialist knowledge required to diagnose her or treat the problem and we don't know what to do. Is there a standard set of tests we should ask for that will diagnose with precision and help guide treatment? Everyone is feeling both scared and helpless.
