Best treatment for Disc herniation at L4-5 level for a 65 year old diabetic?

MRI of Lumbo-Sacral Spine with Screening Full Spine concludes,
1. Disc herniation at L4-5 level with herpertrophy causing mild thecal sac indentation with bilateral neural foraminal narrowing.
2. Disc herniation at L5-S1 & L1-4 level causing thecal sac identation.

The patient suffers from diabetics, hypertension. We have been continuing physiotherapy for last 1 month as the doctor recommended, however for a second opinion we visited another doctor and he said it is not curable with physiotherapy and  the patient needs to do through some sort of operation replacing blood vessels or something. We are a bit scared and tensed to go through any major operation the patient diabetic thus looking for effective alternatives or any other suggestions.
