Do I need surgery?

I have multilevel chronic lumbar disc degeneration and spondylosis with moderate to mild, predominately bony stenosis at L3-4, and to progressively less degrees L2-L3,L4-L5,and K1-2. Stenosis are right dominant from L4 up to L1 and fairly symmetric at L4-5. How serious is this prognosis?
