Just today, for the first time, I have an extreme cold spot on my hand

I have read through your comments and questions and noticed it’s been a few years since someone has commented. Did anyone get any answers?
In the last hour, a spot above my knuckle feels like an ice pack is sitting on it. It comes and goes. It really hurts.

Potential causes of cold fingers include Raynaud's disease, Buerger's disease, peripheral arterial disease, anemia, vitamin B12 deficiency, and hypothyroidism. If your symptoms persist, see your doctor for further workup and evaluation.
