MRI results

Can you some explain my MRI results to me:

IMPRESSION:

1. Transitional vertebral anatomy with a apparent partial sacralization of the
L5 vertebra.

2. The L4/transitional vertebra posterior broad intraspinal protrusion produces
mild impression on the left L5 or S1 nerve within the lateral recess of the
spinal canal.

COMMENT:

Multiplanar multisequence magnet resonance of the lumbar spine are produced for
evaluation.

There is an apparent transitional vertebral anatomy with partial sacralization
of the L5 vertebra. There is generalized decreased T2 signal intensity of the
L4/transitional disc. The disc also demonstrates broad posterior intraspinal
protrusion with noted to a mild impression on the left L5 or S1 nerve within the
lateral recess of the spinal canal.

Hi there!

1. Sacralization refers to an abnormal appearance of the penultimate set of vertebra (lumbar) that makes it appear like/ fused with the last set of vertebral bodies (sacral).
2. An intervertebral disc prolapse is described at L4/ L5, which causes mild compression of the exiting nerve.

Do you think surgery is necessary to fix the prolapsed disc?  I have tried physical therapy, chiropractic, bed rests, medications and it has only gotten worse.
