Regarding My Brain MRI taken 5/15/13

I want to know what Findings and Impression means of my Brain MRI.  My NP said that I had Periventricular white Matter disease.

This is what the MRI says of My Brain:

Findings:  There is no evidence of a midline shift, mass effect,or acute intercranial hemmorage.  The lateral ventricals are normal in size and configuration. The 4th ventrical us midline. There are no abnormal extra-axial fluid collections.  There is no restricted diffusion to suggest an acute infarct. Minimal Periventricular white matter FLAIR/.T2 signal abnormalites.
The cerebellar tonsils are normal in position.  Sella and suprasella regions are unremarkable. The corpus callosum appears normal.
The visualized portion of the globes and orbits are unremarkable. Partial opacification of several ethmoid air cells.  Mild Mucosal thickening of the left sphenoid sinus. Possible tiny air-fluid level in the left maxillary sinus.  Mild paranasal cystic changes. The major intracranial flow voids are symetric.  Mastoid air cells are serated.
IMPRESSION:
1.  No Acute infarct, mass effect or hydrocephalus.
2. Very minimal periventricular signal abnormalities which are non specific in nature, possibly related to chronic microvascular ischemia.
3. Mild Paranasal sinus disease, with possible tiny left maxillary sinus air-fluid level.  ****FINAL REPORT***

I was diagnosed CDC  positive with Lyme Disease in 2009.   I am Chronic now.  I have severe memory loss, sharp stabbing pains, aching pains, arthritis, tremors, anxiety, migraines (which are new) I get headaches but have had about 5 migraines in the last 3 months-which is not normal for me. Headaches are normal.  Etc,  I was wondering if because this Lyme Disease went untreated and was caught too late if it could cause possible Multiple Sclerosis.
My spine hurts constantly, you can't even touch it, that is how painful it is.  All I have been able to do is control the pain the best that I can by seeing a Pain Management Physician (a legit one).  Also, I have recently forgot how to spell and have trouble word finding.  I can go into a room 3-4 times and forget why I went in there,  This scares me as I am only 40 yr old female.  I walk with a cane at times.  I get extremely dizzy for no reason, just sitting at the computer all of a sudden I just get severely dizzy, this has gotten worse lately as well.  I just want to know what is going on with me.

I am waiting for the Neurologist referral to come via the mail. And I pray that he is a good Neurologist.  IF you could help me, with interpreting this MRI, I would be so grateful. Thank you so very much. Oh when I drive I get tremors so bad that I have to use both feet to hold down the brake at a red light..it is embarrassing because I don't want someone to see me shaking like that.

lyme will never go away it is chronic in your case, all you can do is find a lyme literate doctor LLMD that treats only lyme patients.

I have lyme disease and I always will I take antibiotics daily from reading your post I gather you have not had proper treatment for this you should be on at least 3 abx at a time daily twice a day

I have read up on periventricular white matter disease and it is likely that your disturbed vision and balance problems are all associated with that.

It is important for you not to get any stress so that your blood pressure does not go and by doing so will also help to reduce the migraine headaches.  The migraine headaches are related to you having the periventricular white matter disease.  The small seizures that you mention is also because of the white matter disease.

No doubt the specialist will explain everything to you when you see him later in the month.  As you are having sight and balance problems, take someone with you when you see the specialist.  

Can you doctor prescribe you with nicotine patches to help you to cut down on your smoking?

Yes, I know the cigarettes are bad.  The odd thing is that I did have my eyes tested.  Other than having high pressure a couple of times, but it went back to normal.  They are better than 20/20, which doesn't explain the double vision.  I do have the sinus cyst, and I wonder if that may be pressing on my eyes.

I have tried quiting smoking,  hardest thing ever.  I do not drink alcohol. But I think that my Chronic Lyme disease has a lot to do with what may be going on in my brain,

I had a heart cath 2 years ago, and it was perfectly normal, so I don't think it has anything to do with Chonic Microvascular ischemia...although I have inverted Twaves on my ecg.  Who knows,.

I suppose I will get my answers (hopefully) when I see this Neurologist on the 26th of June.  I will be sure to post what his findings are, and What he does.

The odd thing is that I received a second referral from the actual doctor and he wrote on this referral to evalute and treat ( can see him up to 3 times was also written on this one referral)

So, I don't know.  I hate to keep looking it up on line,  I see this mainly in people in their 50's and 60's and not typically in their 40's.  So it is very interesting at the same time scary.

My father has hardening or calcium build ups in his brain -- so I am wondering if this is a cholesterol thing, even though I had that heart cath...oh who knows.

I guess only time will tell.

In the meantime, I have been having like small seizures...I believe that is what they are, and I have had about 6 ot 7 Bad Migraines in my whole life and they were this past year or so.....they don't happen to me normally.  I think they were brought on by stress...but more than a tension headache..But I never had an aura's --that I read about when it comes to migtraines....anyway like I said, I guess I will stick to the professionals.  I was hoping one could answer this question considering the untreated Chronic lyme disease that I suffer from, and a miriad of other things....

Sorry, but I do not know enough about Lymes Disease to make a comment on  that.

But regards your eye sight, I would suggest you see an ophthalmologist to test your eyes and get a prescription for proper glasses.  Although your glasses are 2.50 strength, each eye will be different and the prescription from the ophthalmologist will cater the right strength for each eye.

With regard to cigarettes, cigarettes contain 92 carcinogens and when you smoke you are depleting your brain of oxygen.  For every 7 that you smoke in the presence of a non-smoker, they will have passively smoked 1 cigarette.

Cigarettes also deplete your body of vitamins.

I posted my MRI report, and just kind of want a general idea of what it means as a Nurse Practitioner is whom I see when I go to the doctor's office, I have never once seen the doctor. All I was told was I had periventricular white matter disease.  But it also said that possibly related to chronic micovascular  Ischemia..???  Scared here.  I have memory loss Dizziness, neuropathy on my left side a lot...my leg. foot and
left arm get numb.  I run into everything..loss of coordination. forgot how to spell simple words, which is really strange for me, and also I have a hard time remembering what something is called --thank God for Google or Bing.

So, today I saw my Pain Management docor (was diagnosed with Lyme Disease in July of 2009 by this doctor who is also a family physician, it was CDC Positive for 23 Kd IGM Reactive, 39 Kd IGM reactive, and 41 Kd IGM reactive.  So, it was reported to the Center of Disease Control.  I was given 6 weeks of Doxycycline.  I am now a Chronic Lyme Disease Patient,  Undertreated and getting worse.

On my  diagnostic Bilateral mammogram & Bilateral Ultrasound  that I had the same day as the Brain MRI, I found quite interesting as I had one last year. and every thing was fine.

This year it said on the Ultrasound of my Breast (I had the mammo and ultrasound of my breasts)

Ultrasound said:  Right Breast Ultra sound demonstates a 0.7x0.5x0.3 cm complex cyst at 2:00, and 6cm from the nipple A 0.5x0.4x0.2 cm cyst is present at 10:00 and 2 cm from the nipple.  Left breast ultrasound demonstrates a 0.5x0.5x0.4 cm septated cyst at 4:00 and 3 cm from the nipple.

No solid lesions are identified in the left or right breast.
There is mild bilateral retroareolar ductal estasia.
Bliateral mammograms demonstrated no evidence of malignancy.
Cysts Vary in size and numbers and locations since the prior Ultasound done February 23, 2012. The axillary Lymph nodes were not identified. (I don't know how because my glands in my armpit have been swollen for months)

IMPRESSION:(of US)
1.) Multiple Bilateral Cysts
2.) No solid breast lesions were detected.
3.) Mild Bilateral retroareolar Ductal Ecstasia
1) No mammographic Maliginancy Seen.

Should I at least have a biopsy done. I got all of this multiple breast cysts just since last yeat, I also have on in my sinus, as shown on the Brain MRI report that I already Posted.  Don't you think that the presence of all these cysts is a bit weird and can it have something to do with My Chronic Lyme?

I do have High Cholesterol, which I think would account for the Brain problems, or at least add to the problems.  But don't you think that I should be tested for MS.. MY pain management doctor said that it is Cigarettes and if I keep smoking I am going to turn into a vegetable and not just die, he said that I would suffer..he really freaked me out. I know that smoking is not good for you and that it does cause the veins and arteries to contract, but I do not think it is the reason for my BRAIN MRI results.

Lyme goes into cyst form, I am wondering if these cysts are Lyme Cysts?

I just have so many questions and my doctor's office said that they mailed out the referral but I have not received it at all.  So, I don't know when my appointment is with the neurologist is, or who it is going to be with etc.  

Please I would love a doctor or any one with medical training or anyone whom is Lyme Literate to answer some of my questions.  I am having to close one eye to even see, with out double vision, with reading glasses of 2.50 strength on..SO that is not right,  I shouldn't have to close one eye to see straight,  I would love some ANSWERS>..PLEASE, and Please read the PREVIOUS POST...thank you
