sleep disorders

hello, i am 19  years old ,male
sometimes before sleeping i hear my heart beats weak and when i sleep i feel that i enter a strange state , i feel my breath is stopped and then my head shakes rapidly and sometimes my body or legs shakes and sometimes during this i hear strange voices or dream (see strange things) and in many times i can take a fast breath to return to normal state and sleep

symptoms during day(awake);

  sometimes during awake(in any normal day)(not after these symptoms i mean) :
i feel that i cant breath well and feel numbness in my 4  limbs (both hands and both legs).
,sometimes i feel that i loose my concentration and feel some unbalance and dizzy specially in wide open places it always happens ,i feel strain in head and (same strain i feel but stronger during masturbation )
sometimes i feel something in my head in certain places and when i  touch the place  with my hand i feel some pain ( this sometimes happen before sleeping problems)
i have great problems in digestion   maybe ibs  and also i may have insomnia and cant sleep well
can it just be stress  or Psychological  or sleep disorders?

P.S. Auditory hallucinations are normal when entering the sleep state and are not a sign of psychosis.  :-)

Hey Bud, I am not not a doctor or psychiatrist.  I just remember feeling some of what you are describing when I was your age.  For me, my real issue was spending so much energy worrying about what was going on with me.  Regarding the more intense feelings during masturbation, is there some guilt or reservation you might be experiencing about it which may be adding intensity to your symptoms?  (I believe masturbation is a normal and healthy activity for a young man within healthy boundaries.)  Lastly, do you have a good network of friends?  Sometimes, just going out and doing something fun is the best medicine.  Again, these are just my thoughts.  You just seem a like a cool and normal guy to me.

i want to say that these problems are not in public places and happens at my home
only the head (strain , tension), feeling dizzy and un concentrate  happens in  wide open places but in sometimes happens in closed places !!!!!
can it be problem with balance !!! any problem with cerebellum due to the pain in head mentioned before .(i feel something in my head in certain places and when i  touch the place  with my hand i feel some pain )
i get (dont know the right word but the word opposite to relaxation like anxiety or  tension) too fast and i feel that my body produce adrenaline with great amount
can it be any problem with brain or blood pressure or something like that!!

I sounds like you are having several problems: at night it seems like you have some breathing discomfort and some shakes in your arms and legs, and sometimes some experiences such as hearing voices or strange dreams. It sounds like during waking hours you also have some breathing difficulties and your legs get numb, and it seems your symptoms occur in public places.

Without the ability to review your medical history and examine you personally, I can not give you a diagnosis. However there are a number of possible causes that come to mind and I would be happy to provide you with some information about those.

First, regarding your day time symptoms. A lot of the symptoms you are describing fit into what is called panic disorder with agoraphobia. Basically, agoraphobia is a phobia from social situations, and the result is a lot of anxiety and discomfort in social situations. There is a treatment for this condition.

Your symptoms of breathing discomfort with numbness in all extremities could be caused by hyperventilation: rapid shallow breathing that could be triggered by, for example, a panic attack. When this happens, try to relax. Try to take SLOW and DEEP breaths, and try to breathe into a paper bag, this will help prevent the numbness.

Panic attack and agarophobia are common psychiatric conditions. Patients often describe sensations such as dizziness, a fast-beating heart, light-headedness, numbness in the limbs, a sensation of panic or fear, and sometimes vision changes. They can occur during the day or at night and can obviously cause a lot of discomfort to the person experiencing them.

Shaking of the extremities at night could be caused by many things. The most common one would be jerks of the type seen in the disorder "periodic limb movement syndrome", in which several jerks of the limbs occur within a short period of time. The cause of this condition is not entirely clear, but there is a treatment for it. It is best diagnosed during a sleep study: the patient sleeps under monitored conditions in a sleep center and several recordings are done. Other causes of limb shaking at night include parasomnias, a sleep disorder that includes several not uncommon conditions, and seizures.

Visual hallucinations at night be related to the changes in brain activity that occur as a person falls asleep, transitioning from one state of arousal to another. Their cause may be investigated further during a sleep study.

Before a diagnosis of panic disorder with agoraphobia is made, other causes of your symptoms need to be ruled out, such as problems with your heart or lungs. Also, breathing trouble at night could be caused by a process such as sleep apnea, though your symptoms do not typically fit the latter.

I suggest you see a general practitioner first, then you may benefit from seeing a psychiatrist. If this does not help you, evaluation at a sleep center may also be useful.
