small hand tremors that now turned into many different health issues

I have had a slight tremor in my hands most of my life, but within the past 6 months, other things have made things worse. Let's start with really bad headaches, blacking out when I stand (no fainting), can't remember simple things at times (my address, even though I've lived there for 3 years), head starting to shake (both side to side and up and down), blurred vision at times, getting extremely tired out of the blue (having to pull over to take a nap in my car), hands shaking to the point that my hand writing is messed up, I have difficulty feeding myself without dropping most of my food off my spoon or fork, getting short of breath after doing simple things (walking to the door), walking up 25 stairs, legs shaking, and now big body jerks (like the ones you get when you are falling asleep) but mine are throughout the day in a laying or sitting position, knee jerks (like someone is checking my reflexes), and sever body jerks while sleeping (leaping out of bed almost). What could this be?
