.Severe and ongoing knee pain years after total knee

Had knee replacement surgery 2008 and have had pain since then. Recently it has become debilitating and my ortho guy says my knee films look like a normal knee post-op. Here's the issue when I first began experiencing pain and over the years a total of 3 different orthos have told me I need a revision and that "I'll know when I'm ready" well I'm ready and now my managed care doc and the orthos in the plan all tell me " knee looks normal and they won't approve surgery just because you're in pain"....I've never had managed care before and am quite disgusted with it but am stuck with it until Dec when I can change. There's also a lot of grinding and occasionally the knee will give out when walking.
