Torn meniscus after successful ACL surgery

My son tore his ACL playing soccer in fall of 2017. He underwent surgery with a graft from his own hamstring. Made a good recovery, performed all PT,rehab etc... The surgeon cleared him to begin non-contact workouts. Last week (now July) in a non contact work-out he tears his meniscus in the same knee (verified with MRI). Is this just a freak accident? Or is this normal? Or did the surgeon botch the ACL surgery? I can post all the text of the radiologist reading if it is helpful...
