Bilateral Ovarian cysts post lap

Hello Dr. Goodman,
I recently posted a question about having a mildly elevated ca 125 blood test and the finally diagnosis after having a lap on 4/29. My Gyn found a small patch of endo in the cul-de-sac and lasered it off. After surgery I was told that the mild elevation on the ca 125 blood test and pelvic pain was from this small patch of endo. I recently started having more intense right sided pelvic pain along with lower right back pain. I went back to the Gyn and she did a pelvic exam and said that everything was fine. There were no cysts and that the back pain must be from something else. Well, two weeks later the pain has gotten much worse. I went to my regular doctor who checked my CBC and wanted to do a ultrasound, but couldn't get in for a week for the ultrasound. The pain is getting worse, so I went to the ER. They were able to do an ultasund and stated that I have bilateral ovarian cysts. They appear simple in nature, will a small amount of fluid in the cul-de-sac. The right cyst is 3cm and the left is 4.5 cm in size. Is the size of these cysts concerning considering that my Gyn stated that there were no cysts on 4/29 during the lap procedure? Should I be concerned about these cysts representing something more than simple cysts? Thank you for your time.

Hi Corky
It is reassuring that you had endometriosis and no evidence of malignant change at surgery.  I would assume that the ovarian cysts are either related to ovulation or endometriosis. You should ask your doctor about ovulation suppression such as with birth control pills to help your pain

I just received the actual ultrasound report and it states:

Transabdominal study only performed. Uterus is 9.2 cm in length.
Left ovary contains a 4cm simple cyst.
Right ovary contains a 3.2cm cyst.
Small amount of free fluid in the cul-de-sac is present.
Ap dimension of the endometrial lining is 0.3cm.
Normal arterial perfusion of both ovaries is noted. No evidence of ovarian torsion.

Impression: Bilateral simple ovarian cysts. Small amount of free fluid in the cul-de-sac.

Is this ultrasound report worrisome in light of just having a lap on 4/29 that revealed normal ovaries with no masses? I also had a elevation in the ca125 blood test prior to the surgery and have not had a repeat blood test. My doctor did not feel I needed to repeat the blood test.

Would these "simple" ovarian cysts cause frequent nauseation, pain in the pelvic, hip, and lower back?

Hi There, given your very careful evaluation including a normal laparoscopy in April, simple ovarian cysts are most likely related to ovulation.
Your pain seems more global and makes me wonder about musculo skeletal issues.

With your reassuring  gyn evaluation, you could consider a more holistic approach to feeling better
: exercise, nutrition, centering your energy through yoga or meditation.

You should meet with your pcp to check in about your overall health
