All this means from start to finish is:  your bladder was full at the time of the study (required and normal), the bladder wall appears normal without a mass or stone within it, measurements of your uterus appear normal, it is anteverted / anteflexed which describes how it is positioned in your body (which is also normal), a mid-line echo likely refers to your endometrial stripe which is normal at 5mm, no focal lesion typically refers to fibroids or leiomyomas which were not visualized (normal), then they likely measured your dominant follicle in each ovary which was 4-8mm (also normal).  No free fluid is also normal.

They are probably just watching your follicles to see if your fertility drugs are doing what they are supposed to be doing, stimulating follicles to grow.  Hope this helps.
