COMMENT
TVS performed with consent;
Day 10 of menstrual cycle.
The uterus is anverted and appears sonographically normal in size and shape.
The endometrial lining is thickened (max AP diameter of 13 mm) and heterogenous with echogenic area measuring 5.7 x 6.5 mm and some evidence of a vessel towards this are on color Doppler. This is likely suggesyive of ? endometrial polyps.
Posterior to the uterus and slightly towards the left adnexa there is a large, well defined cystic mass filled with low level echoes measuring 7 x 5 x6.5, there appears to be some ovarian tissue seen surrounding this mass and the impression is that it is likely originating from the ? left ovary. The ultrasound appearance is suggestive of a endometrioma.
A second, large , well defined cystic mass filled with low level echoes is seen, also posterior to the uterus and slightly towards the right adnexa measuring approximately 9.9 x 5.6 x 7.7 cm. There is one septations seen within this cyst.
No vascularity noted within. The ultrasound appearance is again suggestive of a ? large endometrioma. There was no clear ovarian tissue seen surrounding this cyst and therefore difficult to confirm the origin, although likely to be ? right ovarian.The right ovarian could not be seen separate to this cyst.
There is no free fluid seen in the pelvis.
COMMENT: Likely Endometrial polyps. Two large endometriomas posterior to the uterus and extending into adnexae.

Hi...thanks for the answer, i have a question why i have brown discharge 2 weeks before my period is because of my cysts? and i have diarrhoae, sometimes i have fever.

Endometrial polyps are growths of the endometrium that can cause excessive bleeding and pain. They can be easily removed by hysteroscopy and curettage. The endometriomas can lead to pain and backache depending on their location and spread. Large endometriomas will usually need surgery and removal to get relief.
