Hip Pain now CPK over 7000

I am a healthy 43 year old female on no medications.  I have had back & neck problems in the past but nothing serious.  In April of 2007 I experienced several months of severe hip pain.  It eventually decreased with chiro care & exercise.  This past April (08) it started again.  Although I am not sure if the hip pain is realted, I had a CPK test on Aug 1st (my GP ordered this along with thyroid & cortisol tests due to my complaints of fatigue) and it came back at 2500, so I was sent to the emergency room where it tested at 2700.  After many questions and tests, the hospital admitted me & hydrated me with IVs until the level was down to 1700 - then I was released.  As a follow up, my GP ordered another CPK 4 days later & it was up to 7000.  Once again, I went to the hospital emergency room (different one this time - a teaching hospital) and was admitted (when they tested it was 6500 before starting my IV).  I had many many tests performed for my heart, liver, kidneys, thyroid, etc. and all were negative.  I also had an EMG test performed which I passed.  I had started a weight program at the local gym on 7/21 and had 4 sessions (the last one was a rough one) prior to these CPK tests & I am hopeful that this is the problem although the Drs said it should have already started decreasing.  After hydration, my CPK levels were down to 3000 so I was released a 2nd time & told to stay hydrated.  Tomorrow I have to go get another CPK test completed & of course, I am a little afraid...any ideas on what can be causing these high levels without any other real symptoms except some fatigue and hip pain (which has steadily improved since April)???  Thanks.

I've done some searching on elevated CPK levels. And as I'm sure you probably know there are numerous reasons for elevation. Have they R/O (ruled out) Myositis, an inflammatory diseas of the muscles?

I came across an article that talked about pain in the hips. The following 2 paragraphs are from that artice: Begin Quote
"Weakness and pain in the muscles of the hips and shoulders is often a first sign of myositis. You may have difficulty getting up from a chair, combing your hair, or climbing stairs. You may become too tired to walk or stand.
People with a certain form of myositis may also develop a rash on the face, knuckles and other parts of the body. Other possible symptoms include fever and joint pain and swelling. See your doctor right away for diagnosis and treatment." End Quote.

This may not even be a possiblity but when I read "pain in the muscles of the hips" I thought of your symptoms.

CPK is a muscle enzyme.  If the muscle cells are damaged from any cause the CPK enzyme will be released into the blood and the CPK test will be elevated. There are many causes of CPK elevation. Trauma to the muscle including a lot of hard exercise or trauma to the muscle(s) can elevate CPK. Statin's (meds given for high cholosterol) may cause some muscle damage and elevate CPK. But with yours being so very high there must be something going on and apparently that is exactly what they are trying to determine. I have read of even higher CPK levels that were eventually beleived to be due to a virus.

I found the following response to a similar question regarding elevated CPK levels. This response was on MedHelp in 2001. You may want to go to that section of MH and post your question to a Neuro-MD.

Although, I could not find anything in the medical literature regarding twitches and creatine supplements, the CK level of 4000 is NOT normal (if that is really the lab value-decimal points would be very important here). In a research study looking at different lab aspects of creatine users, the highest levels were in th 600's. Elevated CK enzymes can signify primary muscle diseases. I would certainly proceed with the EMG as it can help look at the integrity of the muscle. And stop the creatine now. You will definitely need your labs repeated. The elevated liver enzyme AST can also be due to the creatine, but it is not as concerning as the CK enzyme. Consider seeing a neurologist if the laboratory results don't come back to normal after stopping the creatine. You will also need other blood and urine studies that are routinely done for muscle diseases such as aldolase, lactic acid, and pyruvate. It's possible that everything is just due to the use of creatine, especially if the CK number is wrong. Talk to your doctor about this. Good luck.""""" END QUOTE OF POST
