Child refuses to urinate

I provide Occupational therapy services for a 4 and a half year old child who has (and is) experience neglect in her life.  She is behind on all milestones, needs assistance eating solid foods and randomly hollers things such as, I said no to you, you loser, shut up fatty.  The recent behavior (about 3 months now) is following a UTI that she had.  The child is at our center for 5-6 hours a day and refuses to urinate.  Her stomach by the end of the day is rock hard and she is crying in pain.  She does a lot of pottying dancing by the end of the day to prove she needs to go.  We have tried everything from distracting her with singing, reading books and coloring while on the toilet.  While on the potty she rocks from left to right to hold the urine in.  If we do (very seldom) get her to go she shakes due to the release.  I really want to help this little girl, but don't know what to do next.  I would greatly appreciate any help, Thanks, Trish

We call children's protective regarding this child often.  They actually have a case worker involved in her case and going into the home already.  That was definitely one of the first things done, but unfortunately with her still in the home we want to give her as much support as possible while she is in our care, that is all we can do until they find reason to take her out.  I know this urination piece is her biggest struggle with us right now and it is heartbreaking to see her in so much pain.  According to the doctors there is no UTI although that is where this started in the beginning.  Thanks for helping!

First I would call DFS or Police Department when child is in your care.Second she might still have a Uti.If you know of abuse or neglect you are responable to go to the athouraties.If something bad happens to that child and you knew something wasnt ok in her life and didnt do something you are responsable,and if she is saying things like that,then the abuse probaly is more than verbal.Iam praying that this child gets help and for curage for you to help her.
