phimosis in 4 month old???

ello, I am very worried about my 4 month old son. I took him to our doctor for the first time a week ago. My doctor was concerned about the size of my son's uncircumsized penis hole. He retracted the foreskin back and tried to show me that it was too small, however I was not sure what it should look like so I didn't say anything. He then told me that I need to pull back the forskin at every diaper change and clean with water to help it get bigger. He wants us to go back in a month to make sure it has improved. If not, he said the next step would be creams that will have to be applied daily and if that does not work surgery. Honestly, I feel very uncomfortable with trying to pull back my son's forskin. I have only tried now this week a couple of times, very gentley (not nearly as far as the doctor had). But because i am still afraid to do it, I have been trying to find advice online but I am not finding very acurate information. I plan to get a second opinion BC I think this Dr is very wrong. What do u think?
