Normal blood tests, bilirubin in urine in teenager

Hi, I keep urine dipsticks at home to check me and my family's urine every now and then. For about 10 years now, my son always shows bilirubin in his urine. Everything else is fine, and his CBC and CMP blood test results are always normal. Liver tests are always normal. I read that liver disease usually shows up in the blood test results first, then when it becomes more advanced, it starts showing up in the urine. His results are sort of backwards. Blood tests fine, yet there's always bilirubin in the urine. And like I said this has been going on for 10 years (he is 16 now). No other significant symptoms that he's told me about. He's extremely thin, actually underweight and has been so for at least 6-7 years now. Just doesn't like to eat. Otherwise, eats an extremely healthy diet. Lots and lots of fruits and vegetables, no junk food ever because I don't allow it. And he's home schooled so I know every bite that goes in his mouth. Any insights would be greatly appreciated.
