being a worry wart

Hello everyone.  My son is 20 months old and says several words in english and in spanish but deos not say many 2 word combos. His motor skills are unbelievable.  He climbs, hits balls of the runs jumps throws balls overhard tries to catch, feeds himself tries to brush his teeth ..will mimick things repeats words.. but here is my ?  somedays he is a chatter box other days not.  he says quite a few single words but little 2 word combos.
his receptive language seems ok at times.  He will point to a fish and say fish,  i say pick up your blanket he does, get your milk he does, get your shoes he does, but if i say take this to dad sometimes he will others he wont.  if i say put that back he does not do it.  At times he seems like he just does not understand what i am telling him or maybe does not even care what  i am telling him.
People tell me i am being a paranoid person and not to worry.  His dr. has told me the same thing.. but i wanted to see others imput as well.  Am i being paranoid? or should i be concerned.  I know it seems redundant i am asking this.. but i do not have a huge amount of exposure to children of his age his older brother was much more vocal but not quite as motor.  So my base of camparison is skewd.  I would appreciate any insight

Perhaps he does not understand you when he is busy, reading your thread this is a delightful boistrous boy and sometimes they can be in their own world ,Wow and all those things he does,perhaps because you care so much you are wanting always the best for him, he will get this anyway so back off somewhat and see what unfolds ,sometimes our mothering becomes a little bit" smothering 'You are not paranoid you are a caring Mom so disregard that from others, I am sure they do not mean it either.
