i had sex with my partner a day ago with a condom, i pulled out right after we finished and noticed sperm on top of the head of the penis a little on the shaft and in the reservoir. We did use a water based lubricant, when i looked i had white almost sperm looking wetness on the underside of my penis which to me would be from her and nothing to worry about.  i am worried that some sperm may have come out of the base of the condom, should this be of concern or am i over reacting?

Accidents do happen, though it is not clear anything went wrong here. If she was in the fertile window cycle days 8 thru 16 of 28 she could still take emergency contraception ($50) if you are really concerned. If not in the fertile window, relax! Do you think you need condoms with a bigger reservoir? Could you create that by not putting it on so tightly? For extra protection she could slip in some vaginal spermicide before the action begins. It's up to the two of you to decide how safe you want to be. Best of luck!
