Birth certificate

I live in Indiana does anyone know what my boyfriend needs to bring to the hospital for my child to have his last name?

Im in Chicago I didn't need anything lady came in and just asked didn't need proof

I live in MA. On the paperwork for the birth certificate it needs social security and an ID. Then, we both need to sign an acknowledgement of paternity that the hospital has saying he is the father and takes responsibility for the baby and I agree he is the father of the baby. I would have him bring two or three forms of ID just in case. (Insurance card because it asks for that info somewhere if I remember correctly) License, SS, and insurance cards take up no space in the wallet anyways (and here only mom gets free cafeteria food, dad and everyone else needs to buy their own.)

Most hospitals want a picture id for both the mom and dad. Just call the L&D  ward of the hospital you plan on giving birth in. They will be able to answer your question.

I don't believe my sisters boyfriend had to bring anything with him to the hospital. Pretty sure he just signed something. But I would say his I.d. would be sufficient. We live in Indiana as well.

I live an Indiana too. I havent been told but mine is bringing his I.D. and social security card. I would think that the boyfriend saying yes would be enough but never know!
