HCG levels going crazy.. Anyone Know?

OK bear with me, this is confusing but if anyone has ever had anything similar I would love to hear about it and hear the outcome.

My LMP was 3/6. On 4/6 I got a +HPT. I went the the dr office for them to do a urine test as well on 4/10 and it was "faint but positive". On 4/11, I started very lightly spotting which progressed and got worse over the weekend but never worse than a light period. I had spoken to one of the midwives on call the next night and she basically said if I was miscarrying there was nothing that could be done anyway and I would go sit in an emergency room for hours that night so she recommended I just rest and wait it out if I wasn't in any pain and to call the office on Monday. So that Monday, 4/13, I called the office and explained to them what was going on. I ended up getting sent to the ER anyway. Ended up spending 11 hrs there, got blood work done and an u/s. That night my hcg was at 105 (very low for supposedly 5w) and they didn't see anything at all during the vaginal u/s. I left with the assumption I had miscarried. I got bloodwork done on 4/15 as instructed and it had dropped to 92 and the same day my bleeding stopped. Repeat bloodwork was ordered for 4/23 and it had risen to 156 which I didn't find out until 4/30. I mentioned that I had pain in my left side the night before which I attributed to having pulled something while I was coughing but I thought worth mentioning. I was sent for stat bloodwork and an u/s. My level had risen again to 235 and the u/s showed a cyst on my right ovary that either wasn't there or wasn't mentioned on my trip to the ER. I just had blood drawn again yesterday (5/4) and it was 264. The doctor just called me back a little bit ago to discuss our next step. I've decided to wait it out for a month and get blood repeated weekly.

Now for the part that makes no sense at all and my reason for wanting to wait a month before making a final decision on the Methatrexate. With my last pregnancy (my third), I had pain before I even had a positive office test so they sent me for a blood test. It came back negative (hcg <6). However I was still getting +HPT so I tried a different doctor office. I ended up getting my first appointment at a new office 2 weeks after my neg blood test and at the visit a dating u/s was done and determined I was a little over 6 weeks pregnant.

Now that me, my husband, my family, and my doctor are thoroughly confused, has anyone ever had anything similar with these crazy numbers? What happened in the end?

First, your HCG levels at 5 weeks weren't low. The HCG levels have to start somewhere and gradually increase. Actually sounds very normal for that point in pregnancy.

As for the rise and fall of the levels- it could be ectopic. It could be that you had twins and one died. Your levels are definitely a little off. They need to double within 72 hours.

I think it is wise to see how things progress over the month before taking the methatrexate. Be very mindful of any pains and get U/S stat if you feel weird. If it IS ectopic they need to find it by week 7-8 or you could lose your fallopian tube because it burst.

She sounded pretty confident that it was just a cyst. She too gave me the run down on what to watch for and told me if any of that happens to head straight to the ER.

If it's not ectopic, are chances good that I'll miscarry on my own without needing the shot?

Your numbers are not rising normally - unfortunately this means that this isnt a viable pregnancy.  

A concern is that you might have an ectopic pregnancy - sometimes, slow or abnormally rising HCG levels can indicate this.  Another indicator is one-sided pain.  Was the "cyst" absolutely, positively identified as a "cyst"?

If, during the next month you experience any of the following - persistent or worsening one-sided pain, light headiness, shoulder pain, severe bleeding, call your doctor right away.
