Psa=11.94 after 12 years of prostate removal

My father had a prostate removal from 12 years. After peeing problem he did a PSA test and the score is 11.94 . the doctor suggeestt an MRI and the score was pirads 4 in the  region in contact with.vesicul seminal and pirads 3 in 3 other regions. After that we did a transrectal biopsy echo guided and the result shows no cancer. The doctor is confused and he gave him antibiotic and to redo the psa test after 6 weeks .is that good for now? What do you ssuggest ? Im freaking out.

Not sure I can be much help but it is PSA velocity that matters so he needs another PSA 3 months from last. If rising needs to see urologist again.
