How Bad Is It

On the axial slices of the L1-L2, L2-L3, and L3-L4 into vertebral
disc space no abnormalities are seen.

At the L4-L5 intervertebral disc space diffuse bulging of the
intervertebral disc causing minimal narrowing of both
intervertebral foramen. No spinal stenosis is seen.

At the L5-S1 intervertebral disc space extradural defect
ventrally with a focal area of increased signal on the
T2-weighted sequence consistent with an annular tear is seen. The
finding is consistent with a small central bulge. No spinal
stenosis is seen.

Impression
Bulging of the intervertebral disc at L4-L5 causing minimal
narrowing of intervertebral foramen which is unchanged. No spinal
stenosis is seen. Changes suggestive of an annular tear at the
L5-S1 level as described above. See above comments.

On the sagittal sequences there is narrowing and a small
extradural defect at the C3-C4 and C5-C6 level. No fractures or
dislocations are seen.
At the C2-C3 intervertebral disc space no abnormality is seen.

At the C3-C4 intervertebral disc space extradural defect
centrally and to the left is seen narrowing the left vertebral
foramen slightly consistent with a disc osteophyte formation.

At the C4-C5 intervertebral disc space no abnormalities seen.

At the C5-C6 intervertebral disc space extradural defect
centrally and to the left impinging on the left nerve root and
narrowing the left vertebral foramen consistent with a disc
osteophyte formation is seen.

At the C6-C7 intervertebral disc space no abnormalities are seen.

Impression
Extradural defect centrally and to the left narrowing the left
vertebral foramen at this C4 level consistent with a disc
osteophyte formation is seen. Also at the C5-C6 level extradural
defect centrally and to the left impinging on the nerve root and
narrowing the left vertebral foramen is seen.
