You do not mention any tests that have been done to rule out any conditions.

Have you had your cortisol levels checked to rule out addisons disease?  Have any tests been carried out to rule out conditions such as coeliac disease?  Has your heart been investigated?  Are you constantly tachycardic or does it happen randomly or with postural changes?
